Why at home

At home, medicine understands more.

There are things an office visit can't see: the tricky staircase, the pill bottles on the table, the chair where the day happens. That's why we come to you.

All the care. Pick where to start.

What exactly do you do?

The care you need, where you live.

Two separate services that sometimes cross paths and sometimes don't. Here's which is which, so you know what you're asking for.

Ownership and patient choice

Health Partners Medical Group and Your HealthPartners Home Health Agency are owned by the same people. If you need home health, you choose the agency — you are never required to use ours.

What makes our care different.

01

At home

Care comes to your door

No waiting room, no parking, no transport. We care for you where you live — because that is where medicine understands more.

02

Known by name

You know who cares for you

The same provider, visit after visit. A relationship of continuity, not a different face each time.

03

Transparency

We tell you before, not after

We verify your coverage and explain what applies in your case — no spoken fine print — before the first visit.

04

Your choice

You choose, always

If you need home health and meet the criteria, you choose the agency. And if what you need is something else, we tell you and point the way.

Who we see

If caring for someone has made you feel…

alone.

You're not alone. Health Partners exists for the families who carry that weight — and for those who care for them.

  • 01

They're 65 or older, and leaving the house has become a project.

  • 02

They take several medications no one has reviewed together in a while.

  • 03

They live with high blood pressure, diabetes, a tired heart or lungs, or a memory that is starting to slip.

  • 04

They have a wound that won't finish closing.

  • 05

They just left the hospital and no one really explained what comes next.

  • 06

You're the one who cares for them — and you need someone to pick up the phone.

If you're the one caring

The patient doesn't have to call. Often it's the son, the daughter or the husband doing the looking. Call us yourself: we'll talk, explain what's possible, and decide together where to go from there.

The honest exit

If what you need is something else — a specialist, urgent care — we'll say so plainly and point you in the right direction. Not everything is for us. Telling you that is also care.

Who comes into my home?

Before you open the door, know who's knocking.

Founder · Autonomous nurse practitioner

He qualified as a nurse in 2009 and practiced for fifteen years before completing his master's degree and earning the registration that lets him practice autonomously. He founded Health Partners on a simple idea: the people who most need medical care are often the ones least able to go out and find it.

Advanced practice registered nurse with autonomous practice registration in Florida (license APRN11036456) and family practice certification from the American Academy of Nurse Practitioners. Both credentials can be verified — the license at flhealthsource.gov.

The transparency gesture

In Florida, an autonomous nurse practitioner can diagnose, treat and prescribe in primary care. The law requires us to explain this in writing at your first visit. We'd rather put it on the front page.

Coverage

What does your plan cover? We tell you before, not after.

We work with Medicare and with plans operating in Florida. Coverage depends on your plan and on clinical criteria — so before the first visit, we verify it and explain, in your language, what applies in your case and what doesn't. You decide with the full picture.

And if something isn't covered, we say so right there, with whatever options exist.
